Terms of Use — UTIWizard MyChart

What this service is

UTIWizard MyChart is a patient-facing portal operated by UTIWizard Inc. It lets you connect your own MyChart account so the UTIWizard algorithm can review your past urinalysis results and show what it would have predicted at the time, alongside your actual urine culture results. The service is read-only: it can view your lab results, diagnostic reports, demographics, and medication orders, but it cannot write to, change, or message through your medical record.

Not medical advice

UTIWizard provides clinical decision support for education and discussion with your clinician. It does not diagnose urinary tract infections or any other condition, and it does not replace the judgment of a qualified healthcare professional. Always discuss your results and treatment decisions with your clinician. If you have urgent symptoms, seek care promptly rather than waiting on any prediction shown here.

Your account and access

• You sign in with your own credentials on your health system's login page — UTIWizard never sees or stores your MyChart username or password.
• You authorize which data the portal may read. You can revoke that access at any time through your MyChart account settings.
• You are responsible for keeping your own sign-in credentials and devices secure.

How your data is handled

• Your health data is encrypted in transit and at rest, and access is logged for security auditing.
• We do not sell your health data. We do not use it for advertising.
• We may use de-identified data (with personal identifiers removed) for research and to improve the UTIWizard algorithm. You can opt out of this research use at any time from your portal settings or by contacting info@utiwizard.com.
• We share data only as needed to operate the service (for example, with our cloud hosting provider, which is bound by a business associate agreement) or as required by law.

Acceptable use

You agree not to misuse the portal, attempt to access other users' data, or interfere with the service's operation or security.

Availability and changes

The portal is provided on an "as is" basis. We may update, suspend, or discontinue features as the service evolves, and we may update these terms; continued use after changes means you accept the updated terms.

Limitation of liability

To the fullest extent permitted by law, UTIWizard Inc. is not liable for any decisions you or your clinician make based on information shown in the portal, or for any interruption of the service.
